NoblemanI think it's a pen, but you can't really expect a ref to see that imo.
I would expect a ref or assistant to be able to see every major decision - especially handball on the line from a set piece.
That's just me being in favour of having competent officials though.
There are times when they can't. When someone throws a punch behind the ref's back for example, or does a Zidane, but short of that we should expect the officials to see pretty much everything.
