Hudson Headwaters opens alternative nursing home in Glens Falls
GLENS FALLS, N.Y. (NEWS10)-- Hudson Headwaters Health Network opened the doors of a new alternative nursing home, marking the 10th of its kind in New York State. The Program of All-Inclusive Care for the Elderly (PACE) pays for and provides coordinated medical and social services for people over 55.
PACE helps patients create a personalized care plan coordinated by a team of physicians. It includes primary, dental, home, specialty care, therapies, and prescription drug services.
The new 15,000-square-foot facility on LaGrange Street serves parts of Warren, Washington, and Saratoga Counties. PACE also offers transportation to patient appointments, socialization, and meal services.
The nursing home, which includes an outdoor garden and patio and several spaces for activities and services, was designed to serve as a hub for elderly residents. Officials said the facility is adjacent to Hudson Headwaters' Glens Falls Family Health.
President and Director of PACE Linda Spokane said the PACE model has transformed care for elders and patients' needs. Spokane said a special enrollment team has been made to help participants and their families interested in participating, as they expect a lot of interest in the program.
"There are strict guidelines regarding eligibility," Spokane said. "For example, most PACE participants are duel-eligible, meaning they qualify for both Medicaid and Medicare. Private pay options are also available."
