Grandad took own life after staff ignored 'I'd be better off dead' messages

A Leicestershire grandad took his own life following missed opportunities in his mental health care after staff ignored text messages saying "I'd be better off dead". Steve Dance, 68, was found deceased following a string of failings by NHS staff which led to a delay in getting him the treatment he needed.
