'Cascading, worsening events' led to fatal refinery fire

A federal investigation has concluded a series of “cascading and worsening events” led to the September, 20, 2022, explosion and fire that killed two brothers at Cenovus’ Toledo refinery, the former BP-Husky Toledo oil refinery, in Oregon.
