In Reply We appreciate the Letter and perspective shared by Drs Davis and Lorig in response to our Editorial on the role of lifestyle interventions for obesity in the era of glucagon-like peptide-1 receptor agonists. Although the authors agree with our perspective that there should be multiple strategies for addressing obesity, they call into question the premise that pharmacologic and surgical treatments for obesity are superior to behavioral treatments.
To the Editor We agree with many points made by Dr Ard and colleagues in a recent Editorial. Weight loss is indeed challenging for many people with excess weight. Long-term weight maintenance is difficult to attain, particularly when treatment is withdrawn. However, we challenge the implication that drug and surgical treatments are superior to behavioral treatment. The authors note numerous conflicts of interest in these industries.

There are various criteria of success in the practice of medicine. Some of them are appraised in terms of human psychology, a circumstance that renders them none the less real and legitimate in certain cases. Others are expressed in the restoration of health to those who have consulted the physician; and in such instances the real contribution of the latter is not so easy to evaluate, because of the fact that recovery from injuries of varied sorts is an inherent property of living tissues, and often proceeds without human intervention... This randomized clinical trial examines whether the use of an extracorporeal blood purification device in a nonemergent cardiac surgery population reduces cardiac surgery associated–acute kidney injury after cardiopulmonary bypass.
Acute kidney injury (AKI) is a common complication in hospitalized patients, associated with significant short-term morbidity and mortality, and for which the only current therapeutic strategies are supportive. Of the common causes of AKI, cardiac surgery–associated AKI has been an appealing target for novel therapeutics, in part because the timing of the surgical insult is established and therapies can be offered before, during, or after the surgical procedure. AKI occurs in up to 30% of patients undergoing cardiac surgery. Читать дальше...

A new analysis of recalled cardiovascular devices published in the Annals of Internal Medicine found that most of them hadn’t gone through clinical testing before they were authorized. And when they were, the evidence was usually based on 1 non–randomized clinical trial. The researchers used data from the US Food and Drug Administration (FDA) Class I recalls from 2013 to 2022. There were 137 recalls involving 157 unique cardiovascular devices.

The researchers analyzed data from the 2024 National Youth Tobacco Use Survey, a self-administered, web-based survey conducted among nearly 30 000 middle-school and high-school students.
