In Reply We are grateful to Dr Thadani and colleagues for their interest in our work and applaud their efforts to improve outcomes from AKI around the world. We agree that AKI is important to assess and treat in children with sepsis in any environment and that children in some less-resourced environments are particularly vulnerable to AKI. However, we respectfully disagree with their statement that “shortcomings void the criteria used and perpetuate the neglect of children living in these settings.”

To the Editor We read Dr Sanchez-Pinto and colleagues’ study with interest, but we are concerned that the final Phoenix 4-organ model does not include acute kidney injury (AKI) assessment. Additionally, the Phoenix 8-organ model used only a single serum creatinine threshold. Despite inclusion of data from low- and middle-income countries, these shortcomings void the clinical utility of these criteria and perpetuate the neglect of children living in these settings.

However, an approach involving the annual administration of a monoclonal antibody might provide similar malaria protection while reducing health care visits, a randomized clinical trial found.

Progress in Heart Failure Deaths Reversed Over Past Decade

Although heart failure mortality rates fell between 1999 and 2012, the proportion of people dying from the condition in the US has increased in recent years, according to a study published in JAMA Cardiology. By 2021, the heart failure mortality rate was higher than in 1999, signaling that earlier improvements have been “entirely undone” over the past 10 years, the researchers wrote. The findings were based on death certificate data collected by the US Centers for Disease Control and Prevention.

New Drug for UTIs Gets the Greenlight

The US Food and Drug Administration recently approved pivmecillinam, sold as Pivya, for uncomplicated urinary tract infections (UTIs) in females aged 18 years or older with no structural abnormalities of their urinary tract. Federal Trade Commission Bans Noncompete Clauses

Up to 45% of physicians are subject to noncompete clauses, contractual requirements that can prevent them from changing jobs, which can limit competition and access to care, according to the American Medical Association. However, employers will no longer be allowed to enforce noncompete clauses as of August, when a final rule issued by the Federal Trade Commission (FTC) banning noncompete clauses goes into effect. The move will reduce health care costs by up to $194 billion over the next 10 years, the FTC predicted.

Microneedle Vaccine Patches Generated Immune Response in Children

People who received a microneedle patch that delivered a measles and rubella vaccine mounted immune responses that were equivalent to those vaccinated via subcutaneous injection, according to data from a randomized clinical trial involving 285 participants from The Gambia, including 240 toddlers and infants. The results are the first to support microneedle patches as a safe and effective way to administer the vaccine to children, the researchers noted in The Lancet.

Monoclonal Antibody Authorized to Protect At-Risk People From COVID-19

The US Food and Drug Administration (FDA) granted emergency use authorization to pemivibart, marketed as Pemgarda, to protect people who are immunocompromised from COVID-19. Pemivibart is the only long-acting monoclonal antibody available in the US for preexposure prophylaxis after the FDA revoked authorization of tixagevimab copackaged with cilgavimab, also known as Evusheld, in January 2023.
